Health & Temperament Certification & Daycare Agreement

While group play is a fun activity for our canine friends, there is risk of possible injury during a Daycare setting. OPPR Daycare staff is certified in safe daycare management and strives to keep all our guests safe and happy. In order to ensure that our policies have been explained fully, please read the following:

I understand that by enrolling my dog in the OPPR Daycare program, I am taking certain risks of injury to my dog and/or other dogs. I understand that I am solely responsible for any injury and illnesses that may occur while my dog is attending OPPR Daycare.

I further understand and agree that in admitting my dog, OPPR has relied on my representation that my dog is in good health, is spayed or neutered and has not harmed or shown aggression or threatening behavior towards any person or any other dog. I agree to keep my dog current on all vaccinations that my veterinarian requires.

I understand and agree that OPPR and their staff will not be liable for any problems that develop provided reasonable care and precautions are followed. I hereby release them of any liability of any kind arising from my dog’s attendance and participation at OPPR Daycare.

I further understand and agree that any problems that develop with my dog will be treated as deemed best by the management of OPPR in their sole discretion, and that I assume full financial responsibility for any and all expenses involved.

I am responsible for my dog attending OPPR Daycare at least two (2) times per month, even if my dog only attends Daycare While Boarding at OPPR. Otherwise, I may be responsible for my dog being re-temperament tested with a processing fee of $30.00. This is to ensure that my dog stays compatible with the Daycare group.

I certify that I have read and understand the policies of OPPR Daycare. I agree to abide by these policies and accept all the terms, conditions, and statements of this agreement.
